sentence (I used the Google cache), but according to Google image
search, it has different cover images than the one in my first source
link, albeit unviewable except in miniature because Google doesn't
cache full images. The CC entry gives the title "Forest" for the track,
but everywhere else seems to agree that the title of the track is
"Forest Melody." I'll check the cover tomorrow to see if the track name
holds up.

Sorry about the Donkey Kong thing, TCK. I read that you'd take the Goemon track, but I missed the other part. 
Also, the Chudah's Corner page for the CLANNAD track gives the extended
name of "Nagisa ~ Farewell At the Foot of a Hill," so I assume the
nominator got the track name from there. It's still down,
unfortunately, so you'd have to get the cache for

I'm not bothered about you taking DK64. Hey, I was a bit late as it was, and I'd rather of done research for four than five :P
Re. Clannad: I'll see if I can find a third source, and go with what that says. Same goes for the capitalisation of Clannad.
EDIT: No luck with the third source. I'm tempted to side with Chudah's
on this one, since that's what the track was nominated as. I'll change
it if Kaleb approves.

Chudah's Corner is back up, and it appears that the soundtrack might
have been released by a different publisher. In the "Donkey Kong 64
Official Soundtrack," the track's name is "Forest." In the "Donkey Kong
64 Soundtrack CD," the track's name is "Forest Melody." The CDs appear
identical in every other respect save for album and track names. Your
call for which to use.

Now that Chudah's back up, I looked at the
back of that CLANNAD OST, and Disc 2, Track 16 appears to correspond to
"Nagisa ~ Farewell at the Foot of a Hill".

That gametrax site seems to be the only place
to have Eveline Fischer on the credits for DK64's music. While she
worked on the game, she was just in charge of sound effects. You can
check the last page of the interview with the Rare sound team; Kirkhope takes full credit for the music. I'm going to go ahead and omit Fischer from the credits.

I believe PichuChan ripped this, so I'll just take his word on the
composer names, track title, etc. GameFAQs/IGN/GameSpot listed this as
released in 2002 (both Japan and Europe). Also, some places have the
title listed as "Klonoa Beach Volley", but the European cover clearly says "Klonoa Beach Volleyball", so I'm just going to go with that.

I just added an "Information Format" section to the first comment. This will allow you to forego all that pesky formatting. 
P.S. If you're wondering how I managed to get inline formatting tags,
all I had to do was replace all instances of "[" on the tags with "[color=#FFFFFF][[/color]". Alternatively, you could use "[b][[/b]" if you wanted to make things simpler, and didn't care about the bolding.
